Billing & Insurance

As a courtesy we will bill your health insurance plan (if we have a contract with them). In order for us to do so, you must provide a current copy of your insurance card and your most up-to-date billing information at each visit. Please notify us of any change of address, phone number or insurance provider as soon as possible.

If your insurance company requires a referral from your Primary Care physician in order to be seen, please be certain to obtain this prior to your appointment with us. A delay in obtaining a referral may require us to reschedule your appointment.

Many insurance companies require patients to pay a co-pay or a deductible for certain services. Due to the contractual obligation we have with insurance companies, it is our responsibility to collect your co-pay at the time of your visit. Unless you have made other arrangements with our financial office, if you are unable to pay your co-pay at the time of your visit, we will need to re-schedule your appointment.

If you do not have insurance, we do request payment on the day of service. We offer a cash discount for patients who pay in full at the time of visit. Our billing department and financial office are informative and willing to assist you.

Payment Methods:
Triad-Obgyn accepts the following forms of payment: Cash, Checks, Money Orders, VISA, Master Card and Debit Cards. You will be responsible for any bank fees associated with any checks returned to us for non-payment. any account over 90 days will be issued to small claims as a means to collect payment.
